Every price on the page is a floor
Maximus lists eight medications and prices all of them the same way. Compounded semaglutide is "Starting at $116/28-day supply" and compounded tirzepatide "Starting at $198/28-day supply". The brand drugs follow the same pattern: "Starting at $199.99/mo" for Foundayo tablets, "Starting at $249.99/mo" for Wegovy tablets, "Starting at $349.99/mo" for Wegovy pens and "Starting at $449.99/mo" for Zepbound.
Not one of those figures is tied to a dose, and the page carries no table of what the doses cost. So there is no price on this page we can record as what treatment costs — only the least you could pay. The banner across the top offers "$100 OFF first month of medication for new clients purchasing 12-month plan", which is the shape of the whole site: the attractive number is attached to the longest commitment.
A 28-day supply is not a month
The compounded plans are priced per 28-day supply, and the brand ones per month. Those are different units. A year holds thirteen 28-day periods, not twelve, so a compounded plan bills one more time a year than a monthly plan at the same headline rate. The page sets the two unit types side by side without remarking on it, and the cheaper-looking figures are the ones on the shorter cycle.
The commitment runs both ways, and only one way is fixed
Maximus's Subscription Savings Plan asks participants to "prepay for services for a specified term (e.g., 3, 6, or 12 months) at a discounted rate". Three clauses in its terms have to be read together.
You may cancel mid-term, but you "will remain responsible for fulfilling the payment obligations for the remainder of the commitment term" — so cancelling stops the treatment, not the bill. The plan "automatically renews at the end of the term for successive terms of the same duration at the then-current rate", with at least 30 days' notice of the new rate before it does. And separately: "Changes in treatment care during the commitment term may result in higher or lower monthly charges depending on the prescribed care plan."
Put together, that is a term you are locked into, at a price that can move inside it. If your dose goes up, the charge can go up with it, and the commitment does not release you.
What is not refundable
Almost everything. The terms state that fees "are due immediately when charged by Maximus and are non-refundable", and that "this no refund policy shall apply at all times". Payments for recurring membership periods after the first are not refundable. Where an exception is granted, laboratory testing and clinician consultation fees are "strictly nonrefundable, as these services are rendered at the initiation of clinical care". There are two narrow openings: a full refund minus the consultation cost if you are not approved for treatment, and a 48-hour window on a Labs subscription provided no testing has happened.
Missing an appointment costs money too — a "no-show fee of $20", and a "$20 short notice reschedule fee" for moving an appointment inside 24 hours.
Two of the drugs are not weight-loss drugs
The same page lists Ozempic and Mounjaro alongside the weight-loss medicines. Both are labelled for type 2 diabetes rather than for weight management, which is a distinction the page does not draw. It matters for what a prescriber can write and what a pharmacy will fill.
